• formularz zgłoszenia  (register)
• oświadczenie o zobowiązaniu do uczestnictwa w projekcie (declaration of participation in the project)
• oświadczenie o akceptacji regulaminu  (declaration of conforming to the rules)
• oświadczenie o wyrażeniu zgody na przetwarzanie danych osobowych   (agreement to process personal data)
